Pursuant to the Communiqué on the Principles and Procedures for Application to the Data Controller,
the Data Subject must include in the application their name, surname, signature if the application is in
writing,
Turkish ID number (or passport number if the applicant is a foreign national), residential or workplace

within no later than 30 days from the date they reach us.
